The season 9 premiere of Supernatural is only three weeks away and the CW has released a second trailer for the inaugural episode.

While the sneak peek and the previous promo for "It Think I'm Gonna Like It Here," where both a little vague in terms of discussing Sam's (Jared Padalecki) illness, the new video gives fans quite a bit to think about.

We now know that at least part of the episode will take place inside the younger Winchester's mind, a la season 6's "The Man Who Knew Too Much." However, this time Dean (Jensen Ackles) is there as well, a feat he seems to have pulled off with a little help from Ezekiel  (Tahmoh Penikett).

So, how much of the episode is taking place inside Sam's grapefruit and how much is based in actual reality? It looks like Sam's argument with Dean regarding his reasons to keep fighting, Sam's encounter with Death (Julian Richings), and the reappearance of Bobby (Jim Beaver) will take place in his own internal universe. Meanwhile, Castiel's (Misha Collins) trouble with another fallen angel, Dean's experiences in the hospital, and Abaddon's (Alaina Huffman) threats will all be occurring in real time.

With that being said, the trailer stops short at providing fans with the source of Sam's actual cure. It seams highly unlikely that the entire season will be based inside Sammy's noggin, so there must be some way to heal him although it undoubtedly will come with a high cost.

Supernatural returns to the CW on Oct. 8.
