Britney Spears is "out of control" and needs medication and a new conservatorship, a celebrity psychiatrist has suggested.

During a Monday appearance on "TMZ Live," Beverly Hills-based board-certified psychiatrist Dr. Charles Sophy was asked about his thoughts on Spears' situation more than two years after she was freed from her 13-year conservatorship under her father Jamie.

Sophy -- who specializes in adult psychiatry as well as child and adolescent psychiatry -- said he believes Spears has a "mental illness" but may not be taking medication after regaining control of her personal life and finances.

"The conservatorship was there for a reason. Even though... it might not have been with the right person, the actual process needs to be in place," the Hollywood psychiatrist explained. "If you have a mental illness to this degree, you need that kind of structure, supervision, and most importantly, to take your medication."

Sophy added that if Spears were under his care, the first thing he woud do to help her would be to give her medication, even if he had to inject it to her.

Describing Spears' recent actions as a "no medication" situation, Sophy pointed to her "erratic behavior, manicky spending of money, [and] recklessness" as potential signs that she's not getting help.

The doctor went on to claim that Spears appears to be "destroying the people around her" to the point where "no one feels safe around her."

"She's out of control on many levels," he claimed of the pop superstar.

Spears has been plagued by controversies since the termination of her conservatorship.

The singer repeatedly left fans concerned over her well-being with her social media posts, which included a video of her dancing with two knives and a dance routine that depicted her choking herself.

Spears' longtime partner Sam Asghari filed for divorce from her in August 2023 after only one year of marriage. TMZ later reported that the "Toxic" hitmaker allegedly attacked Asghari as he slept and left him with a black eye and bite marks.

TMZ also recently reported, citing unnamed insiders, that Spears is dangerously unstable and in "serious danger" of going broke after being given sole access to her $60 million estate in 2021. But unnamed insiders told Page Six that Spears is "not going broke" despite her alleged lavish spending.

Sophy shared three potential scenarios that could happen if Spears continues to go allegedly without medication and supervision.

The psychiatrist predicted that Spears could hurt herself, harm another person, or be placed on a 5150 psychiatric hold if she's not able to take care of herself. He said that any of these scenarios could lead to Spears being given medication "against her will."

While he acknowledged that another conservatorship may be unlikely considering the "public outcry" over her first one, Sophy suggested that it might be necessary to keep Spears "stable."

"At the end of the day, hopefully a judge would make the decision in the best interest of that patient. And if [a new conservastorship] is what she requires, even for a temporary amount of time, that's what she would require," he told "TMZ Live."

Last week, Spears settled her conservatorship case with her father for an undisclosed amount.

TMZ reported, citing unnamed sources, that Spears was required to pay Jamie's legal bills, which allegedly totaled more than $2 million.

The legal battle centered on Jamie's legal fees and his handling of his daughter's finances.

Spears' lawyer had alleged that Jamie "reaped many millions of dollars from Britney as a conservator, while paying his lawyers millions more," USA Today reported.

But Spears' estranged dad claimed in a 2021 petition that he was entitled to have his legal bills paid by the singer "to ensure the Conservatorship can be wound up quickly and efficiently to allow Britney to take control of her life as she and Jamie desire."
