Modern Family star Sarah Hyland was granted a restraining order against ex-boyfriend Matthew Prokop last week.

The restraining order - which requires Prokop to stay 100 yards away from the actress and her dog - was filed in response to his exit from rehab on Sept. 21, TMZ reported. The rehab facility's director called Hyland and recommended she take action because of his mental state.

In court documents obtained by the website, Hyland, 23, also chronicled the abuse she faced from Prokop, whom she dated for almost five years. In May, the 24-year-old actor reportedly threw Hyland against a car during an argument, cursed at her and aggressively grabbed her neck.

"His grip was so tight that I could not breathe or speak. I was scared and in fear for my life," Hyland said, adding that she had a "very sore throat following this event."

The documents also state that Hyland asked her television mother Julie Bowen for help. She asked Bowen to "come to her house to help peacefully end the relationship." Hyland also bought her ex-boyfriend a plane ticket so he could head home to Texas.

When Prokop arrived at Hyland's house that day, however, he reportedly became aggressive when he saw Bowen and "ran outside into the backyard and began screaming." He also threw a lighter at Hyland. Bowen intervened and told her co-star to leave the house for safety.

But, that's not all. Prokop said he would burn one of Hyland's homes down and even implied he'd harm her dog. And, after she broke up with the actor in August, Hyland said he "relentlessly bombarded me with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats."
