Episode 18 of Grey's Anatomy, titled Idle Hands, airs Thursday on ABC and fans will not only see Derek Shepard struggle to appease his pregnant wife, but also physically struggle with a patient following a verbal dispute.

A sneak peak clip from the upcoming episode, released by the network, showed Shepard and one of his interns approach a man named Mr. Kramer in the newly named Grey Sloan Memorial Hospital waiting room and tell him that his daughter is alive. The man, however, seemed visibly upset by the news and grabbed the intern's shirt, trying to assault him. Shepard got involved and began pushing Mr. Kramer off of the intern. ABC titled the sneak peak clip "Fight preview," which has fans guessing that a physical altercation will take place in the episode. See the clip below.

A promo video for episode 18, shown below, revealed that Shepard goes "McCrazy" over baby troubles. Not that anything is wrong with his child, but his wife, Meredith Grey, turns out to be his "biggest challenge in his life." Grey is a pregnant, paranoid and hormonal wife. In one scene the couple was in bed and she told Shepard maybe their child has 11 toes.

"McDreamy could go McCrazy," the clip suggested. In Idle Hands the couple also learns the sex of their baby and whatever the gender is made Miranda Bailey very happy, as she was seen shrieking for joy.

Bailey tried to impress Cristina Yang with "cutting-edge medical technology," but in the end Bailey cannot get the support or money she needs for her new project. Jackson Avery also continued to ignore Bailey's questions, concerns and suggestions. He yelled at her "I am a doctor today, Bailey!"

An official synopsis released by ABC for Idle Hands stated that Alex Carev "give's Jo's new boyfriend a run for his money," perhaps in light of his jealousy, " and Callie Torez and Arizona Robbins "try to get the spark back in their relationship."

The hospital continued to make changes under a new name and management, and the doctors began to realize "owning a hospital comes with its own set of problems and responsibilities."

Episode 18 of Grey's Anatomy, part of its ninth season, airs Thursday, March 21, at 9 p.m. ET on ABC.
