Concerning details regarding Joan Rivers' death were released Monday revealing that she wasn't handled with proper care during her throat procedure over the summer.

According to a new report from the Centers for Medicare & Medicaid Services (CMS), an unnamed patient with the same description and date of surgery as Rivers was treated at Manhattan's Yorkville Endoscopy clinic with major errors.

For example, the clinic didn't weigh the patient before the surgery, which is necessary for proper sedation, and didn't pay attention to "deteriorating vital signs and provide timely intervention." Additionally, the clinic didn't get her permission for each procedure performed that day.

The former Fashion Police host faced complications during the throat surgery performed in New York City on Aug. 28. Rivers went into cardiac arrest during the procedure and was rushed to Mount Sinai Hospital. She was kept on life support in the days that followed and passed away on Sept. 4 at the age of 81.

The comedian, whose career spanned almost 50 years, suffered from hypoxia during the throat procedure, which meant there was no oxygen traveling to her brain, TMZ reported in October. In turn, she suffered from brain damage.

Yorkville Endoscopy released a statement to address the new report. "In response to the statement of deficiencies, Yorkville immediately submittted and implemented a plan of correction that addressed all issues raised. The regulatory agencies are currently reviewing the corrective plan of action and have been in regular contact with Yorkville," it reads. "In addition, the physicians involved in the direct care and treatment referenced in the report no longer practice or provide services at Yorkville."

And, Rivers' daughter Melissa "is terribly disappointed" by the news. "As any of us would be, Ms. Rivers is outraged by the misconduct and mismanagement now shown to have occurred before, during and after the procedure," a statement from her attorney reads.
