A worker tragically died in a blender at a meat plant Friday. The grisly accident took while 41-year-old Hugo Avalos-Chanon was cleaning inside the plant. However, he reportedly fell into an operating meat blender.

The accident happened at 11.45 p.m. and despite work colleagues pushing the emergency stop button as soon as they realized what was happening, the man had already suffered fatal injuries. Paramedics were called to the scene but they were unable to save his life.

The body reportedly suffered "blunt-force injuries and chopping wounds", according to deputy state medical examiner Dr. Cliff Young.

The blender had to be dismantled by firefighters to remove the body.

The incident has been described as a tragic "industrial accident" and no foul play was suspected.

The plant was reported to have machines that were not locked during the cleaning process, according to a report last fall by the Occupational Safety and Health Administration (OSHA).

The OSHA has already started an investigation and spokeswoman Melanie Mesaros said, "It's way too early to say (if anyone is to blame). We're just starting our investigation, which could take six months."

Witnesses have been answering questions to investigators and safety records are being reviewed at the plant.

The plant had not violated any rules in the past, according to Mesaros.
