The website notes that at "Derek's BMI Office," someone named Dr. Smith speaks about gene receptors. Dr. Smith views a scan and says he's never seen such a case before.

The spoiler also reveals that a female patient named Nova has a big crush on Dr. Smith and calls him a "fine doctor." Nova's critical surgery also has been postponed again.

According to WetPaint, Dr. Smith may even be an alias for Derek.

Before the show's winter finale, Derek turned down an offer to partake in a prestigious study for Meredith.

But, after he received a phone call from the President of the United States to join him on a brain mapping project, it may be impossible for him to deny the proposition. That one phone call may have the potential to tear the couple apart for good.

The synopsis for the episode, titled "Take It Back," explains that "Meredith feels betrayed when Derek goes back on a promise."

Sarah Drew, who plays April on the show, previously revealed how much of an impact that one phone call will have for the couple's relationship.

"I can tell you that everything kind of gets turned upside emotionally because of the President's phone call to Derek," Drew told Glamour. "All kinds of things get messed up because of that phone call."
