The Yorkville Endoscopy outpatient clinic in which Rivers' health troubles were first reported is under investigation, according to a new CNN report. The comedian was undergoing a minor throat operation when she started having respiratory problems and going into cardiac arrest.

She was later moved to the Mount Sinai Hospital in New York where she has been held for a week until her death Thursday afternoon.

The New York medical examiner office "will be investigating the death of Joan Rivers," spokeswoman Julie Bolcer told CNN. Reportedly, the New York State Department of Health is currently "investigating the matter," according to spokesman James O'Hara.

The Yorkville Endoscopy website describes the facility as "state of the art" and "staffed by highly experienced endoscopists whose mission is to provide safe and compassionate care to patients and their families."

It is also reportedly "approved by the Department of Health," but as CNN notes, the description of its accreditations are non-existent.

According to Rivers' daughter, Melissa Rivers, she died at 1:17 p.m. in Mount Sinai where she was surrounded by family and friends.

"My mother's greatest joy in life was to make people laugh," Melissa continued in the statement. "Although that is difficult to do right now, I know her final wish would be that we return laughing soon."

Joan was 81 years old.
